DESCRIPTION:On behalf of Innovative Integrated Health (IIH)\, the operator of our new Orange County PACE facility\, I would like to invite you to our Ribbon Cutting Ceremony and Open House Reception on Thursday\, December 9\, from 1:00 pm to 5:00 pm\, with the Ribbon Cutting Ceremony taking place at 1:30 pm.\n\n\n\nOur Orange County PACE facility is located at 1125 N. Magnolia Avenue\, in the City of Anaheim\, just north of La Palma Avenue.\n\n\n\nAs you know\, PACE stands for the Program of All-Inclusive Care for the Elderly.  PACE provides a comprehensive preventative health program along with social and wellness services for frail\, low-income seniors in your community.  It is designed to be the "one-stop shop" for eligible seniors in your city to receive primary preventative and acute medical care\, as well as social\, recreational\, nutritional and personal care services.  The goal is to keep our seniors happy\, healthy and living independently in their own homes and not be institutionalized in long-term care facility.\n\n\n\nOur PACE-eligible participants will come from our Orange County PACE service area which includes most of the north and central Orange County area.\n\n\n\nThere will be a host of community and distinguished guests in attendance (including a number of elected officials) and the event should be a fun\, social and celebratory one!\n\n\n\nLight refreshments will be served and we will also be conducting specialized tours as part of our Open House throughout the afternoon to showcase our new facility.
